The Transition project is based in Southampton supporting young people in Year 6 to transfer smoothly to secondary school.

Our aim is to tackle the core issues of the transition from primary to secondary education through a dedicated team of staff engaged with the issues, needs and aspirations of the young people referred to the project. The project supports not only the young person, but also the whole family to ensure that all aspects and concerns around transfer are addressed. The young people will be supported to engage in a range of activities that will enable them to improve their social skills, and access wider community provision.
